Acute and Chronic Bronchitis

Bronchitis is an inflammation of the bronchial tubes — the airways that carry air into the lungs. It is one of the most common respiratory conditions worldwide and presents in two distinct forms: acute bronchitis, which is short-lived and usually follows a viral infection, and chronic bronchitis, which is a long-term condition and a component of chronic obstructive pulmonary disease (COPD). Understanding the difference between the two is essential for appropriate management and care.

What Is Acute Bronchitis?

Acute bronchitis develops rapidly, typically as a complication of a cold or influenza virus. The bronchial lining becomes inflamed, causing a persistent cough that may produce mucus, mild fever, chest discomfort, and fatigue. Most cases resolve within two to three weeks without medical intervention, though the cough can linger for several weeks after other symptoms clear.

  • Primary cause: Viral infection (rhinovirus, influenza, respiratory syncytial virus)
  • Duration: Typically one to three weeks
  • Contagious period: While the causative virus is active, usually the first few days
  • Common symptoms: Cough with or without mucus, low-grade fever, chest tightness, mild breathlessness

Bacterial causes of acute bronchitis are less common and account for only a small proportion of cases. Antibiotics are generally not recommended unless a bacterial infection is confirmed, as overuse contributes to antimicrobial resistance.

What Is Chronic Bronchitis and How Is It Different?

Chronic bronchitis is defined clinically as a productive cough — one that brings up mucus — occurring on most days for at least three months of the year, for two consecutive years, when other causes have been ruled out. Unlike acute bronchitis, it is not caused by a single infection. Instead, it results from persistent irritation and damage to the airway lining over time.

The most significant risk factor is long-term cigarette smoking, which is responsible for the majority of cases. Prolonged exposure to air pollution, occupational dust, chemical fumes, and secondhand smoke also contributes. Over time, the bronchial walls thicken, mucus glands enlarge, and the airways narrow, making breathing progressively more difficult.

  • Duration: By definition, at least three months per year for two consecutive years
  • Primary risk factors: Smoking, air pollution, occupational dust, secondhand smoke
  • Associated conditions: Emphysema, COPD, recurrent respiratory infections

How Is Bronchitis Diagnosed?

Diagnosis of acute bronchitis is primarily clinical — a doctor reviews symptoms and medical history, listens to the lungs, and rules out pneumonia. No specific laboratory tests are routinely required for uncomplicated acute bronchitis. For chronic bronchitis, spirometry (a lung function test) is the standard diagnostic tool, assessing how much air a person can exhale and how quickly.

A chest X-ray may be ordered to exclude other conditions such as pneumonia or lung cancer. In some cases, sputum analysis helps identify specific bacterial infections that may require targeted treatment.

What Are the Treatment Options for Bronchitis?

For acute bronchitis, treatment focuses on symptom relief. Rest, adequate hydration, and over-the-counter remedies for fever and cough are usually sufficient. Steam inhalation can ease airway congestion. Honey has demonstrated modest evidence for soothing cough in adults and is a widely used home remedy.

Chronic bronchitis management is more complex and aims to slow progression, reduce symptom burden, and prevent exacerbations:

  1. Smoking cessation — the single most effective intervention for slowing disease progression
  2. Bronchodilators — inhaled medications that relax the airway muscles and improve airflow
  3. Corticosteroids — inhaled or oral steroids to reduce airway inflammation during exacerbations
  4. Pulmonary rehabilitation — structured exercise and education programmes to improve quality of life
  5. Vaccinations — annual influenza vaccine and pneumococcal vaccine to reduce infection risk

Traditional Ayurvedic Approaches Used in Sri Lanka

Sri Lanka has a centuries-old tradition of Ayurvedic medicine, and several indigenous herbs and preparations are used in the management of respiratory conditions including bronchitis. While these remedies should complement rather than replace conventional medical care, many have documented anti-inflammatory or antimicrobial properties.

Ceylon black pepper (Piper nigrum) is one of the most recognised spices with expectorant properties, traditionally used in herbal decoctions to loosen mucus and ease coughing. It is commonly combined with ginger and honey in folk preparations for respiratory complaints.

Cloves (Syzygium aromaticum) contain eugenol, a compound with demonstrated anti-inflammatory and antimicrobial activity. In Ayurvedic practice, Syzygium aromaticum is sometimes used in steam inhalation blends or herbal teas intended to soothe irritated airways.

Formulations such as Yogaraja Guggulu and Kaishore Guggulu are classical Ayurvedic compound preparations used by practitioners for inflammatory conditions. They are typically prescribed by qualified Ayurvedic physicians rather than used as self-treatment. Similarly, Pushyanuga Churna is another traditional herbal formulation employed within the Ayurvedic system for respiratory and other inflammatory conditions.

Ceylon green tea, rich in antioxidant polyphenols, is widely consumed across Sri Lanka and may offer mild anti-inflammatory support for the respiratory tract. Ceylon Green Tea is produced primarily in the hill country estates and is a staple of daily life on the island.

When Should You See a Doctor?

Most cases of acute bronchitis resolve without medical treatment, but certain symptoms warrant prompt medical attention. Seek care if a cough persists beyond three weeks, if there is high fever, blood in sputum, significant shortness of breath at rest, or if symptoms worsen after initially improving. These could indicate pneumonia, asthma, or another condition requiring specific treatment.

For anyone with a chronic cough and a history of smoking or significant environmental exposure, formal medical evaluation — including spirometry — is strongly recommended, even if symptoms seem manageable. Early detection of COPD allows interventions that meaningfully slow disease progression.

Lifestyle Measures That Support Respiratory Health

Regardless of whether bronchitis is acute or chronic, several lifestyle habits support better airway health and resilience against infection:

  • Avoid smoking and secondhand smoke in all settings
  • Use a mask or respirator in dusty or chemically hazardous environments
  • Stay well hydrated to keep mucus thin and easier to clear
  • Practice regular hand hygiene to reduce viral transmission
  • Maintain a balanced diet rich in antioxidants from fresh fruit and vegetables
  • Engage in moderate regular exercise, which supports lung function over time
  • Ensure indoor spaces are well ventilated, particularly in kitchens and rooms with solid-fuel stoves

In Sri Lanka's tropical climate, high humidity can sometimes worsen respiratory symptoms. Travellers or residents with existing chronic bronchitis may find certain highland areas such as Ella or the cooler hill country more comfortable for breathing than the humid coastal lowlands, though individual responses vary.
